Custom-designed questionnaire service that creates a complete research instrument aligned with your research objectives. The questionnaire includes: Section A - Demographic Profile (age, gender, education, income, etc. as relevant), Section B - Main Questions using a mix of question types (5-point Likert scale items, multiple choice, ranking questions, semantic differential scales, open-ended questions), and Section C - Validation Questions. Questions are sequenced logically from general to specific, avoiding leading, double-barreled, and ambiguous items. The service includes sample size guidance using Krejcie and Morgan table or Cochran's formula, a Google Form version for online distribution, and a Word version for print distribution. Also provided: a cover letter template, instructions for pilot testing, and reliability analysis (Cronbach's alpha) once you collect 20-30 pilot responses.
